DOT Drug and Alcohol Testing Information Release

All applicants for BELCO TRANSPORT, INC. must provide the following information and meet the qualifications listed to be considered for employment:

1. Must be at least 23 years old.
2. Have at least 2 years of verifiable interstate experience driving solo.
3. Must have a valid commercial driver license.
4. Must provide DMV printout of driving record dated within the past
   30 days. Driving record must meet the following guidelines:
   • No more than 2 moving violations in past twelve months, or
     5 moving violations during the past 3 years.
   • No more than once accident in the past 3 years.
   • Applicant cannot have any of the following violations:
     1. DUI
     2. Driving without license or while license is revoked/suspended.
     3. Reckless driving
     4. Speeding in excess of 20 mph of posted speed limit.
     5. Leaving the scene of accident
5. Must have valid Medical Exam and provide a copy of Long Form
   with application.
